2026 Free Throw Championship Entry Form

The Saints Simon & Jude Knights of Columbus will hold the 2026 basketball Free Throw Championship for all boys and girls ages 9 through 14 on two dates: Friday, January 16th at 9:00 PM and Saturday, January 17th at 5:00 PM. The participant’s age category is determined by his/her age as of Jan. 1st. Contestants register either at the door by filling out a brief entry form or download form, fill out and bring to the event at the Saints Simon and Jude School gym. A parent or guardian’s signature is required for entry. Each age/gender category winner receives a regulation-size basketball and Winner’s Certificate. Winners progress to the District competition on Saturday, February 7th at St. Agnes parish where other awards are provided.
